Drinking Soda – Risk For Pancreatic Cancer

ScienceDaily (Feb. 8, 2010) — Consuming two or more soft drinks per week increased the risk of developing pancreatic cancer by nearly twofold compared to individuals who did not consume soft drinks, according to a report in Cancer Epidemiology, Biomarkers & Prevention, a journal of the American Association for Cancer Research.

Although relatively rare, pancreatic cancer remains one of the most deadly, and only 5 percent of people who are diagnosed are alive five years later.

Mark Pereira, Ph.D., senior author on the study and associate professor in the School of Public Health at the University of Minnesota, said people who consume soft drinks on a regular basis, defined as primarily carbonated sugar-sweetened beverages, tend to have a poor behavioral profile overall.

However, the effect of these drinks on pancreatic cancer may be unique.

“The high levels of sugar in soft drinks may be increasing the level of insulin in the body, which we think contributes to pancreatic cancer cell growth,” said Pereira.

For the current study, Pereira and colleagues followed 60,524 men and women in the Singapore Chinese Health Study for 14 years. During that time, there were 140 pancreatic cancer cases. Those who consumed two or more soft drinks per week (averaging five per week) had an 87 percent increased risk compared with individuals who did not.

No association was seen between fruit juice consumption and pancreatic cancer.

Pereira said that these results from Singapore are likely applicable to the United States.

“Singapore is a wealthy country with excellent health care. Favorite pastimes are eating and shopping, so the findings should apply to other western countries,” said Pereira.

Susan Mayne, Ph.D., associate director of the Yale Cancer Center and professor of epidemiology at the Yale School of Public Health, said these study results are intriguing but have some key limitations that should be considered in any interpretation.

“Although this study found a risk, the finding was based on a relatively small number of cases and it remains unclear whether it is a causal association or not. Soft drink consumption in Singapore was associated with several other adverse health behaviors such as smoking and red meat intake, which we can’t accurately control for,” said Mayne, an editorial board member of Cancer Epidemiology, Biomarkers & Prevention.

Pereira points out that the findings are biologically plausible, held up in non-smokers, remained similar after taking other dietary habits into account and are consistent with findings in Caucasian populations.

Glycogen Metabolism and Glycogen storage Diseases

GLYCOGEN:
Glycogen is the major storage carbohydrate in animals, corresponding to starch in plants; it is a branched polymer of α-D-glucose. It is stored mainly in liver and muscle. Due to more muscle mass, the quantity of glycogen in muscle (250gm) is about 3 times higher than that in the liver (75gm).

Functions:
a. Liver glycogen helps to maintain blood glucose level:
Liver glycogen functions to store and export glucose to maintain blood glucose level between meals. After 12-18 hrs of fasting, liver glycogen is almost totally depleted.

b. Muscle glycogen serves as a fuel reserve for the supply of ATP during muscle contraction.

c. Although muscle glycogen doesn’t yield free glucose (as muscle lacks Glucose-6-phosphatase), pyruvate formed by glycolysis in muscle can undergo transamination to alanine, which is exported from muscle and used for gluconeogenesis in liver.

Structure:
- homopolysaccharide composed of α-D-glucose
- glucose units are held together by α-1,4 glycosidic linkages
- α-1,6 glycosidic linkages at branching

GLYCOGEN METABOLISM:

Glycogenesis and Glycogenolysis Pathway

a. Glycogenesis: The synthesis of glycogen from glucose occuring in the cytosol in the presence of ATP, UTP and glucose is called glycogenesis.

Steps:
1. Glucose is converted into glucose-6-phosphate by the action of glucokinase or hexokinase.
2. Glucose-6-phosphate is converted into glucose-1-phosphate by the action of Phosphoglucomutase, passing through an obligatory intermediate step of glucose-1,6-bisphosphate.
3. Glucose-1-phosphate is converted into UDP-glucose by the action of Uridyl Transferase (also called UDP-glucose pyrophosphorylase) and pyrophosphate is formed, which is hydrolyzed by pyrophosphatase into 2 molecules of Pi.
4. Glucose molecules are assembled in a chain by glycogen synthase, which must act on a pre-existing glycogen primer or glycogenin (small protein that forms the primer). The mechanism for joining glucose units is that glycogen synthase binds to UDPG, causing it to break down into an oxonium ion, also formed in glycogenolysis. This oxonium ion can readily add to the 4-hydroxyl group of a glucosyl residue on the 4 end of the glycogen chain.
5. Branches are made by branching enzyme (also known as amylo-α(1:4)->α(1:6)transglycosylase), which transfers the end of the chain onto an earlier part via α-1:6 glucosidic bond, forming branches, which further grow by addition of more α-1:4 glucosidic units.

Overall reaction:
(Glucose)n + Glucose + 2ATP = (Glucose)n+1 + 2ATP + Pi

b. Glycogenolysis: The degradation of stored glycogen in liver and muscle constitutes glyogenolysis. It is not the reverse of glycogenesis but is a separate pathway.

Steps:

1. First step:
The overall reaction for the 1st step is:
Glycogen (n residues) + Pi = Glycogen (n-1 residues)+ G1P

Here, glycogen phosphorylase cleaves the bond at the 1 position by substitution of a phosphoryl group. It breaks down glucose polymer at α-1-4 linkages until 4 linked glucoses are left on the branch. Glycogen phosphorylase can be used as a marker enzyme to determine glycogen breakdown.

2. Second step:
The 2nd step involves the debranching enzyme that moves three remaining glucose units to another 1,4 end of glycogen. The final action of the debranching enzyme is the hydrolysis of the glucose attached as a 1,6-branched mono residue, giving one free glucose molecule. This is the only case in which a glycogen metabolite is not glucose-1-phosphate.

3. Third step:
The 3rd and last stage converts G1P (glucose-1-phosphate) to G6P (glucose-6-phosphate) through the enzyme phosphoglucomutase.

Degradation of glycogen by lysosomal acid maltase:
- Acid maltase or alpha-1,4 glucosidase is a lysosomal enzyme that continuously degrades a small quantity of glycogen.
- Deficiecy results in glycogen accumulation causing Pompe’s disease.

Mechanisms of regulation of glycogenesis and glycogenolysis:
1. Allosteric mechanism
2. Hormonal mechanism
3. Influence of calcium

1. Allosteric regulation:
The control is carried out in such a way that glycogen synthesis is increased when substrate and energy levels are high. On the other hand, glycogen breakdown is enhanced when glucose concentration and energy levels are low.

2. Hormonal regulation:

Hormaonal regulation of glycogen metabolism

- The hormones, through a complex series of reactions bring, about covalent modification : phosphorylation and dephosphorylation of enzyme proteins.
- cAMP acts as a second messenger for hormones.

3. Influence of calcium:
- Calcium ions are released from sarcoplasmic reticulum during muscle contraction
- Calcium binds to calmodulin (calcium modulating protein) and directly activates phosphorylase kinase without the involvement of cAPM-dependent protein kinase.
- Muscle phosphorylase kinase activates glycogen phosphorylase.

Increased glucagon or epinephrine level = increased glycogenolysis
Increased insulin = increased glycogenolysis

GLYCOGEN STORAGE DISEASES (GSD):
The metabolic defect concerned with the glycogen synthesis and degradation are collectively referred to as glycogen storage diseases. Glycogen storage diseases are inherited.

Common Types:

Type I: von Gierke’s disease

Enzyme defect: Glucose-6-phosphate
Organs involved: Liver, kidney and intestine
Features: Glycogen accumulation in liver and renal tubule cells; enlarged liver and kidney; hypoglycemia; lactic acidemia; ketosis, hyperlipemia; gouty arthritis

Type II: Pompe’s disease
Enzyme defect: Lysosomal α-1,4 glucosidase (acid maltase)
Organs involved: All organs
Features: Glycogen accumulation in lysosomes; juvenile onset variant: muscle hypotonia, death from heart failure by age 2; adult onset variant: muscle dystrophy; enlarged liver and nervous system is also affected.

Type III: Cori’s disease or limit dextrinosis
Enzyme defect: Amylo α-1,6 glucosidase (debranching enzyme)
Organs involved: liver, muscle. heart, leukocytes
Features: Accumulation of branched chain glycogen; liver enlarged; hypoglycemia; muscle weakness.

Type IV: Andersen’s disease or amylopectinosis
Enzyme defect: Glucosyl 4-6 transferase (branching enzyme)
Organs involved: Most tissues
Features: Hepatosplenomegaly; liver cirrhosis; accumulation of glycogen with few branches; death from heart or liver failure by age 5.

Type V: McArdle’s syndrome
Enzyme defect: Muscle glycogen phosphorylase
Organs involved: Skelteal muscle
Features: Muscle glycogen stores very high; poor exercise tolerance; muscle cramps; blood lactate and pyruvate very low after exercise.

Type VI: Her’s disease
Enzyme defect: Liver glycogen phosphorylase
Organs involved: Liver
Features: Hepatomegaly; accumulation of glycogen in liver; mild hypoglycemia; ketosis.

Type VII: Tauri’s disease
Enzyme defect: Muscle and erythrocyte phosphofructokinase 1
Organs involved: Skeletal muscle, erythrocyte
Features: Muscle cramps due to exercise; blood lactate not elevated; hemolytic anemia; poor exercise tolerance.

Changes!

So I bit the bullet and changed my layout. Not that the other one was bad, in fact I loved my blog layout. BUT I tend to get bored of the same thing over and over, and I really was in need of some cleaning of my blog widgets, pictures and sidebar info. I love the professionalism that this layout brings, as well as the subdued color scheme.
I hope you all enjoy it, too!

I tweeted this morning that I might be changing my blog name, as well. For now I am keeping it. Sam said it right – It’s catchy! Yes, I think it is pretty catchy and since everyone knows me as The Clean Eating Mama… why change a good thing. Ya know?!

I had a lovely bowl of cold cereal this morning for breakfast:

A mixture of granola, roasted almonds and some other cereal that was taking up space in our pantry. I love mixing cereal – the different flavors and textures all add something special to what is normally called a “boring” bowl of cereal.

I made J his own bowl of goodness, too.

Ten Days as a Lean Mean Clean Queen

Ed left this early morning for a business trip to San Francisco for ten days.  (Sad.)  He’s actually going to be about 30 minutes away from most of my Mom’s side of the family – wish I could have tagged along!  With finishing up this semester of school and starting training for my new job, the timing was just not gonna cut it.  Bummer!  However, I’ve decided to make good use of this mandatory Me-time: Strict clean eating for ten days.  (If you don’t know clean eating and Tosca Reno yet, you should.)  We had a pretty indulgent weekend with friends and I am feeling sluggier than usual this Monday morning, so I’ve decided to scour all of my past issues of Oxygen magazine and whip up a week of planned-in-advance healthy meals, tailored to my ideal calorie level and macro breakdown.

This means I got to indulge myself in some healthy grocery shopping!  Does anyone else look forward to shopping for food?  (I have grocery issues – eh, just lump that together with my scone and coffee issues.)  We’ve been on a tight-ish budget recently, which meant we’ve whittled down our pantry, fridge, and freezer foods to a respectable emptiness, so I didn’t feel too bad for stocking up.  It was refreshing to replenish my clean eating staple foods.  Today’s grocer choice was Trader Joe’s – and I got some great deals!  Impressive.  Seeing my fridge fully stocked, I already feel more motivated to work toward my goals.

Speaking of which, motivation.. This is what is fueling my next ten days: “A goal without a plan is just a wish.” Amen to that! I had a higher-calorie-than-planned weekend, but I enjoyed it, and it’s not the end of the world. All I can do is clean it right back up and fire up some refreshed goals. Ed’s family is planning a July 4th BBQ-Pool Party.. I want to feel comfortable in my swimsuit and be able to wholly enjoy myself.  The ultimate goal is a two-piece bathing suit for Ed and Dr. Ben’s Half-Ironman in August. 

My ten-day healthy meal plan is processed-food free, which means I will be prepping a lot this week.  Get out your tupperware!  I stocked up on more varied protein sources than just the chicken, whey, tuna, and eggs I usually eat – ground turkey, wild salmon, shrimp, low-sodium lean roast beef, firm tofu, edamame, and dried beans will show up in my menu this week.  (Hopefully this will vanquish any boredom I might start to feel.)  I also varied up the regular veggie choices – nix’ed the baby carrots for the duration, opting instead for asparagus, brussels sprouts, bok choy, celery and red onions.  I will also eat bigger breakfasts and smaller dinners.  I (and a lot of people, I find) tend to eat the other way around – a quick bowl of oatmeal in the morning and then way too much food, even it’s healthy stuff, too close to bedtime. 

I’m not going to give myself strict rules, and it might not be exactly what Tosca would advise (seen above.. sidenote that I just noticed.. what the heck is she mixing?  looks like black leaves?  hmm.. any thoughts on that?); I’m just aiming for some adjustments to my eating patterns and variety to my choices.  (Notice I never used the word ‘diet?’)  With Ed away, I won’t be tempted to add sauces and cheese and extraneous carbs to our shared meals.  That’s fine in moderation for maintenance, but let’s see if I can lean out a bit more as summertime approaches.  I’ll keep you posted on progress, and any fun finds along the way!

Baked Tahong

1 ½ kilos tahong, boiled and half shell removed
1 pouch (115 grams) hot and spicy pizza sauce
10 cloves garlic, minced
1 tbsp snipped parsley
50 grams quick melt cheese, coarsely grated
2 tbsp bread crumbs

Preheat oven to 350*F
Arrange tahonh on a baking tray. Top with pizza sauce and remaining ingredients. Bake until cheese melts

A Peek at Tuesday’s Killer Workout: 925 Calories DOWN.

Ok, I already did today’s blog post (below this one) but I wanted to share a peek of the workout combo I did today. I know that I personally have discovered a LOT or most of my favorite home workout DVDs or types of workouts (i.e. kettlebells or Tabata) from other bloggers. So hopefully by sharing my “reviews” many of you can discover some new favorites.

Cathe has ALWAYS been a favorite. This woman knows her stuff. Her latest workout DVDs are killer, offering a ton of high intensity cardio. So I did Core Cardio Circuit first- stopping before the cooldown and then going right into Legs & Glutes, skipping the warmup for that and doing probably 80% of the video.

SWEATFEST. I CRUSHED IT. 925 CALORIES BURNED! WOOT!

Champagne, Beer & Alcohol


So many of my clients ask me about alcohol and how it affects their weight loss and weight training. While I was shopping around for a friend to find the perfect bottle, I noted that champagne in the UK is on sale – bigtime. Perhaps its the economy and people are just not celebrating as they used to with the bubbly, but Champagne in the UK is cheap! All the prices below are in Sterling and full bottles!

Tesco’s Premier Cru – just 14.98
Pierre Darcy (in Asda) – just 10.00
Louis Chaury (Marks & Spencer) – just 12.99

I couldn’t believe my eyes. And champagne is better on the belly than other alcohols. Haven’t you noticed that many thin women prefer to drink champagne? Well it is a secret aid to staying lean. This is only if you really want to have a drink.

Jillian Michaels in her book, The Final Cut, says that for the final 10 lbs, we all have to give up the drink. Total abstinence. It’s good for the body and mind as well to do without the poison for a while. Alcohol dehydrates the body. Not smart for someone on a good diet looking to add muscle and burn fat.

Each glass of champagne (4 fluid ounces) is approximately 95 calories. That’s rather low for an alcoholic drink. For example, a Yuengling bottle of beer is 125. For me, that is at least 5 minutes less on the treadmill!

An interesting fact to note is that there is no nutritional value to a drink. Calories and that is it. Nothing of value elsewhere and you are adding calories to your diet but not feeding the body or feeling full. So you will still eat the same amount you usually do (plus the calories from the alcohol.)

But now I know why my friend Vicki J at Home House can drink champagne and still has biceps bigger and more defined than most of the other men in the bar! I will just have to make a trip to Tesco’s to find the cheaper Champagne!

Is Food Combining Real Science?

Have you heard of “food combining”? The basic concept is not to mix carbohydrates with proteins or fats as they interfere with each other during digestion.
I have always had digestive issues in spite of the fact I am a light eater and stay clear of big fatty meals.  Even as a kid, a burger or plate of meat and potatoes left my belly bloated for hours.
I have tried all kinds of digestive supplements to see if I was intolerant, but nothing helped.
Then it dawned on me that maybe it wasnt the food it was the mixture of foods I was eating. I for example I can eat a bagel no problem but ad bacon and egg and I bloat. Chicken ceasar salads are no problem but add croutons-you get it.  I google this and its called food combining.  I mentioned this to my doctor and he thinks its quackery. It also doesnt seem to be as mainstream as the other food fads. Your thoughts appreciated! — Tony G.

Dear Tony,
Yes, I agree with your doctor. This bogus myth has been around for a hundred years, yet people have been eating sandwiches for decades! There is nothing in our physiology that prevents combining any foods together. The hormones and enzymes in our stomach and small intestine respond to each type of food (carbohydrates, fats, or protein) regardless of other types entering the digestive system at the same time.
There is a chance you have some type of digestive intolerance and it may be to gluten or it may be to fats… or it may just be that adding another food results in too much volume for you to comfortably digest at one time.
Try eating whatever you like in smaller portions (like 1/2 bagel with bacon and egg) and see if this helps!
